1.) Origin Services Invoice (booking, packing, etc.)

• Original GBL
• Weight Tickets
• Original DD619
• 1299 and Orders
• Inventories

2.) Line haul Services Invoice (Line haul, ATC, etc.)

• Copy of GBL
• Weight Tickets
• DD619-1
• 1840
• Inventories (direct delivery)

3.) Destination Services Invoice (SIT, warehouse, etc.)

• Copy of GBL Reweigh tickets 
• DD619-1  SIT Certificate
• 1840 Inventories

IMPORTANT: 

• Include on your invoice the Shipper Name, GBL number and Registration Number.
• The T.O must sign the DD619-1 for SIT, Reweigh, and Labor charges.
• Include long delivery from SIT Orders and SIT Extension notices when applicable.
• Be sure to include Diversion Orders and GBL Correction notice that amend the GBL.
